The following explains how to get your visit summary through the desktop website. If you are on a mobile device, please go here.
If your provider wrote a work/school release note, you will find it here. If it is not in the visit summary, please contact your provider. They may not have uploaded it yet.
Step 1. Log into eVisit and click on the down arrow next to your name.
Step 2. Click on “Visit History”.
All your previous visits will be displayed, the most current visit will be at the top.
Step 3. Find the date of the visit you are looking for, then click “Download” which will be on the right side of the screen.
A PDF of your visit summary will be downloaded onto your computer. Take note of where your downloads go so you can find it later.
If you are looking for specific information and cannot find it on your visit summary, please contact your provider directly to request it.
